Anatomic Pathology: Hematopathology

942) Which of the following is characteristic of this lymphoma?

• The flow cytometric dot plots show a population of cells co-expressing dim CD20, CD19, CD5, and CD23. Chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) is characterized by the following immunophenotype: CD19 positive, CD20 positive (dim), CD5 positive, CD23 positive, CD43 positive/negative, FMC7 negative to dim, and dim surface light chain immunoglobulin expression.

• CLL/SLL typically follows an indolent clinical course (C). t(8;14) is typical of Burkitt lymphoma (BL) and can be seen in other aggressive B-cell lymphomas (B).

• BL is CD20 bright, CD10 positive, Bcl6 positive, CD43 positive, CD5 negative, and Bcl2 negative, while t(11;14) is seen in mantle cell lymphoma, which is CD20 bright, CD5 positive, and CD23 negative.

• Histologic examination, CLL/SLL will show vaguely defined, pale-looking areas rich in paraimmunoblastic/prolymphocytic cells known as proliferation centers/pseudofollicles.
