Anatomic Pathology: Thyroid and Parathyroid Gland Pathology

911) A 73-year-old woman underwent total thyroidectomy. Histologic examination of the gland revealed an incidental papillary carcinoma that showed prominent nuclear grooves, as shown in the photograph. Which of the following tumors also display(s) nuclear grooves?

• The nuclear features of papillary thyroid carcinoma include enlargement, irregular contours, chromatin dispersion/nuclear clearing, grooves, and pseudoinclusions containing cytoplasmic material.

• Nuclear grooves sometimes occur in nonneoplastic contexts. In the thyroid, the nuclei of oncocytes can show grooves.

• Many other tumors characteristically demonstrate nuclear grooves. Grooved nuclei are typical of solid pseudopapillary tumor of the pancreas, and they represent a diagnostic feature of chondroblastoma.

• Among ovarian tumors, Brenner tumor and the adult type of granulosa cell tumor display nuclear grooves.

• Bronchioloalveolar carcinoma/pulmonary adenocarcinoma in situ sometimes shows nuclear pseudoinclusions and grooves. These features may be useful in terms of distinguishing this lesion from benign reactive atypia.
