945) In the post-transplant setting, which of the following T/natural killer (NK)-cell lymphomas are the MOST common posttransplant lymphoproliferative disorders (PTLDs)?

• Hepatosplenic T-cell lymphoma and peripheral T-cell lymphoma NOS are the most common T/NK-cell posttransplant lymphoproliferative disorders (PTLD). T/NK-cell PTLD makes up only 15% of all PTLD.

• Diagnosis of a T/NK-cell PTLD is based on diagnostic criteria for T/NK-cell lymphomas/leukemias seen in immunocompetent patients. Unlike B-cell PTLDs, only a minority (about a third) of T/NK-cell PTLD are associated with Epstein Barr Virus (EBV).

• The pathophysiology of T/NK-cell PTLD for most cases is unknown. In addition, T/NK-cell PTLD arises typically at longer intervals after the start of immunosuppression and has a worse prognosis compared to B-cell PTLDs.

• Extranodal NK/T-cell lymphoma (A), adult T-cell leukemia/lymphoma (ATLL) (B), and anaplastic large cell lymphoma (ALCL) (E) have all been reported in the transplant setting; however, they are not the most common T/NK-cell PTLD. Angioimmunoblastic T-cell lymphoma (AITL) (D) has not been reported in the transplant setting.
