Anatomic Pathology: Thyroid and Parathyroid Gland Pathology

912) A 40-year-old woman underwent fine-needle aspiration of a 2.4 cm nodule in the right lobe of her thyroid. The findings were consistent with papillary carcinoma, and she underwent total thyroidectomy. Histologic examination of the surgical specimen resulted in a diagnosis of hyalinizing trabecular tumor (HTT). Among thyroid tumors, this lesion exhibits a unique staining pattern with which of the following immunohistochemical markers?

• Hyalinizing trabecular tumor (HTT) consists of trabeculae and nests within a delicate fibrovascular stroma.

• This tumor features nuclei with prominent grooves and cytoplasmic pseudoinclusions, and consequently mimics papillary carcinoma in fine-needle aspiration specimens.

• HTT characteristically demonstrates circumscription and is sometimes encapsulated. These features may facilitate distinction between HTT and medullary carcinoma in some cases.

• HTT characteristically demonstrates circumscription and is sometimes encapsulated. These features may facilitate distinction between HTT and medullary carcinoma in some cases.

• Like many other thyroid tumors, HTT generally shows positive staining with thyroglobulin and thyroid transcription factor-1 (TTF-1), and negative staining with calcitonin.
